Social determinants of health (SDoH) are linked to diabetes and prediabetes prevalence, poorly controlled diabetes, and cardiometabolic disease disparities leading to national calls to address upstream SDoH to improve health equity. In order to act upon SDoH – either through tailored care delivery or social referral outside of the health system – clinicians must first be able to screen for adverse SDoH, or social risks.

The audience will learn strategies to identify individuals with diabetes and link them with local resources within their practice site workflow. They will also receive social determinants of health screening training to increase knowledge and comfort with identifying and addressing individualized patient needs.

The purpose of the Alabama Infectious Diseases Society (ALIDS) is to promote strong antimicrobial stewardship throughout the state of Alabama.  This meeting in-person meeting will provide educational sessions on different topics related to antimicrobial stewardship.
