Social determinants of health (SDoH) are linked to diabetes and prediabetes prevalence, poorly controlled diabetes, and cardiometabolic disease disparities leading to national calls to address upstream SDoH to improve health equity. In order to act upon SDoH – either through tailored care delivery or social referral outside of the health system – clinicians must first be able to screen for adverse SDoH, or social risks.

The C.A.R.E.S. continuing education programs are designed for Alabama pharmacists and pharmacy technicians to provide an overview of Medicare and the roles of the Aging and Disability Resource Centers (ADRCs) with the primary goal to help pharmacists and pharmacy staff effectively and efficiently assist Medicare beneficiaries. All of our CE programs are funded by the Alabama Department of Senior Services (ADSS) and the U.S. Administration for Community Living. They are made available to pharmacists and technicians in Alabama at no cost.

This course will equip the audience to identify and address social needs in patients with diabetes. The audience will learn strategies to identify individuals with diabetes and link them with local resources within their practice site workflow. They will also receive social determinants of health screening training to increase knowledge and comfort with identifying and addressing individualized patient needs.
