Social determinants of health (SDoH) are nonmedical factors that significantly influence health outcomes. Studies highlight the need and opportunity for healthcare providers to screen for negative SDoH and connect individuals with resources to address these risks. This presentation will review SDoH, their prevalence in Alabama, and their impact on people with diabetes. It will also cover the current status of screening and referral practices, as well as considerations for incorporating SDoH-related services into practice.

Sleep disorders have bi-directional relationships with many other co-occuring conditions; they may both impact and be impacted by these co-occuring conditions and related pharmacotherapy. Management approaches are limited overall, and become more limited when considering certain patient specific variables.
