2023 APhA Delivering Medication Therapy Management Services

APhA’s Delivering Medication Therapy Management Services certificate training program presents a systematic approach for developing, implementing, delivering, and sustaining MTM services. It includes an overview of the marketplace for delivering MTM services, guidance for implementing MTM services in pharmacy practice, a review of the essential skills and knowledge needed for performing MTM successfully, and an organized process for identifying medication-related problems. The purpose of this certificate training program is to prepare pharmacists to improve medication use through the delivery of MTM services in a variety of practice settings.

This ACPE activity does not provide a certification in this topic but rather advanced professional training which upon successful completion the learner will be able to download a certificate of achievement.

Auburn's Harrison College of Pharmacy (HCOP) is an APhA licensed partner.  A HCOP instructor will facilitate APhA's activities related to this program. Participants will register for the program through this HCOP CE website, which will provide the information to access and complete program materials through APhA's website. 

This certificate training program is made up of three components – the 10hr self-study, the 8hr live training, and the post live activity.  The pre-course work is required before the live training date which will be held via Zoom on August 10, 2023.  Please read through the instructions within this course.  All of the home study coursework will be completed through the APhA site linked within the course materials.  The link to the Zoom webinar will be included within the course and also emailed to you.

P. David Brackett, PharmD, BCPS is the Coordinator of Clinical Services and Residency Program Director at Auburn University Clinical Health Services (CHS).

He provides direct patient care to CHS patients and provides clinical and administrative oversight to the clinicians at Alabama’s State Wellness Center and provides leadership for the clinical and educational services offered throughout CHS.

Dr. Brackett has been a pharmacist, clinician and educator since 1996. Dr. Brackett has provided clinical pharmacy services for numerous patients especially those with chronic medical problems such as hypertension, diabetes, heart failure, anticoagulation, hyperlipidemia, asthma/COPD and GERD.

Dr. Brackett received a B.S. in biology from Jacksonville University in 1990 and his Doctor of Pharmacy from the University of Florida in 1995. He then completed an ASHP-accredited Pharmacy Practice Residency at Shands at UF.

Dr. Brackett, his wife Kim and their two sons, Brendan and Kyle, live in Auburn. He is active in the community as an Assistant Scout Master for Boy Scout Troop 15 and as an usher and lay minister for Holy Trinity Episcopal Church.

Accreditation Information

The American Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. The APhA Delivering Medication Therapy Management Services certificate training program is approved for a total of 21.0 contact hours of continuing pharmacy education (CPE) credit (2.1 CEUs). The ACPE Universal Activity Numbers (UAN) for this activity are listed below.

Successful completion of the self-study component involves passing the self-study assessment with a grade of 70% or higher and will result in 10 contact hours of CPE credit (1.0 CEUs). 0202-0000-21-224-H04-P

Successful completion of the live seminar component involves attending the full live seminar and completing the online evaluation. Successful completion of this component will result in 8 contact hours of CPE credit (0.8 CEU). 0202-0000-21-225-L04-P and 0202-9999-21-225-L04-P

Successful completion of the post seminar case exercise component involves completing 3 post cases and submitting an online attestation statement of completion. Successful completion of this component will result in 3 contact hours of CPE credit (0.3 CEU). 0202-0000-21-226-H04-P

Successful completion of the post seminar case exercise component for Minnesota pharmacists involves completing 12 post cases and submitting an online attestation statement of completion. Successful completion of this component will result in 12 contact hours of CPE credit (1.2 CEU). 0202-0000-21-227-H04-P

To obtain 21.0 contact hours of CPE credit (2.1 CEUs) for APhA's Delivering Medication Therapy Management Services program, the learner must complete all components listed above, and CLAIM credit for each component. Participants will need to have a valid APhA (pharmacist.com) username and password, as well as a CPE Monitor account to claim credit. After credit has been claimed, please visit CPE monitor for your transcript. The Certificate of Achievement will be available online upon successful completion of the necessary activity requirements on the participant’s My Training.

APhA continuing pharmacy education policy provides you with two opportunities to successfully complete the continuing pharmacy education assessment. Please note that you will not be permitted to submit the assessment a third time. The current policy of the APhA Education Department is not to release the correct answers to any of our CPE tests. This policy is intended to maintain the integrity of the CPE activity and the assessment.

Release Date: July 2, 2021

Expiration Date: July 2, 2024 - PLEASE NOTE: NO Home Study credit granted after this date; Live Credit can only be granted within 60 days from the day of the seminar attended or before the expiration date, whichever is sooner.

 

 

Activity Completion Requirements

To obtain credit for this learning activity, you must successfully complete the activity and have a current APhA account. Your transcript of CPE will be on your CPE Monitor account within 24-hours of successful activity completion. If you are licensed in Florida, your CPE will be filed with CE Broker. 

A current APhA account which includes

  • NABP e-Profile ID and PIN
  • Florida Pharmacist / Pharmacist Technician license number (only applicable to Florida Pharmacists and Pharmacy Technicians)
  • Access your APhA My Account to verify the above information.  

 

Credit for the live seminar portion of this certificate training program will be earned upon completion of home study materials, full attendance of live seminar, successfully passing injection technique assessments, completion of the live seminar online evaluation, and achieving a passing grade of 70% or higher on the final assessment. To obtain credit, the Learner must CLAIM credit once each component is completed. Once credit is claimed, Transcripts of CPE will be available online within 24 hours on the Learner’s CPE Monitor profile.  The Certificate of Achievement will be available online upon successful completion of the necessary activity requirements on the participant’s “My Training” page on www.pharmacist.com.
