Studies highlight the need and opportunity for healthcare providers to screen for negative SDoH and connect individuals with resources to address these risks. This presentation will review SDoH, their prevalence in Alabama, and their impact on people with diabetes. It will also cover the current status of screening and referral practices, as well as considerations for incorporating SDoH-related services into practice.
