Recorded Screening for and Filling Gaps in SDoH: Addressing Social Determinants for Better Outcomes in Alabama
Social determinants of health (SDoH) are nonmedical factors that significantly influence health outcomes. Studies highlight the need and opportunity for healthcare providers to screen for negative SDoH and connect individuals with resources to address these risks. This presentation will review SDoH, their prevalence in Alabama, and their impact on people with diabetes. It will also cover the current status of screening and referral practices, as well as considerations for incorporating SDoH-related services into practice.

Courtney Gamston is a 2013 graduate from the Harrison College of Pharmacy. After spending a year with the Auburn University Pharmaceutical Care Center as an ambulatory care resident, she accepted a faculty position at the Edward Via College of Osteopathic medicine as Assistant Professor and Discipline Chair for Pharmacology. She currently also holds the position of Ambulatory Care Clinician with the AUPCC.
Along with serving as a Ambulatory Care Clinician in the Clinical Affairs and Outreach unit, she also serves as a Professor of Practice for experiential learning. In this position, she participates in the Practice-Ready Curriculum with Pharm.D. students, including in small and large group instruction, active delivery of educational content, and assessment of pharmacy related skills. She also collaborates with other healthcare professionals to provide model care to patients in an interdisciplinary environment, utilizes an electronic health record to document all patient care activities, identifies patients for pharmacist led interventions using population health metrics, and precepts Pharm.D. students in both active and simulated patient care activities throughout all four years of the pharmacy curriculum.
Her current research focuses on establishing, improving, and maintaining sustainable ambulatory care services in a pharmacist-led primary care clinic. Areas of focus include prediabetes, diabetes, obesity, dyslipidemia, and hypertension. Gamston's research is focused not only to the provision of medication therapy management but also on patient education services that improve self-care behaviors and overall health. Another facet of this work is improving the education of pharmacy and DO students in the realm of patient education and medication/disease state management in the ambulatory care setting.
The goal of this research is twofold: (1) to establish models of ambulatory care practice for implementation in a variety of settings and (2) to enhance the education and experience of pharmacy and DO students in order to prepare them to operate independently in an ambulatory care setting.
