Chronic coronary disease (CCD), formerly known as "stable ischemic heart disease," encompasses a diverse range of conditions, including both obstructive and nonobstructive coronary artery disease (CAD), with or without a history of myocardial infarction (MI) or revascularization. It also includes ischemic heart disease diagnosed solely through noninvasive testing and chronic angina syndromes arising from various underlying causes. Approximately 20 million people in the United States live with CCD, and it remains a leading cause of death.

Research has shown that practitioners trained in Motivational Interviewing (MI) have greater confidence in talking with individuals about new topics and greater satisfaction with worklife due to increased success in helping facilitate an individual's decision-making for behavior change. Social determinants of health (SDoH) are nonmedical factors that significantly influence health outcomes. Studies highlight the need and opportunity for healthcare providers to screen for negative SDoH and connect individuals with resources to address these risks. This presentation will review SDoH, their prevalence in Alabama, and their impact on people with diabetes. It will also cover the current status of screening and referral practices, as well as considerations for incorporating SDoH-related services into practice.
